Discover how a health assessment can become a personal blueprint, bringing priorities and recommendations together in a plan for life at home.
EPISODE 01
Understanding your health is a starting point. The next step is turning that knowledge into habits that fit your everyday life.
In the first episode of Conversations by ZEM, Dr Carlos Gutiérrez and Xilenia Zoghbi explore a practical approach to Mediterranean Longevity: knowing where you stand and building changes around the life you lead.
Knowing your strengths, priorities and areas that need attention helps shape a personal health plan. Carlos emphasises that professional guidance should consider health as a whole, while taking your routine and surroundings into account.

Dr Carlos Gutiérrez
Clinic Director, ZEM Wellness Clinic Altea
Mediterranean eating should be enjoyable and possible to maintain, rather than an ongoing sacrifice. Seasonal ingredients, minimally processed foods, cooking and shared meals all feature in the discussion.
At Finca Althaya, these ideas connect with understanding where food comes from and spending time outdoors.
Walking a little more, making time to pause and nurturing meaningful relationships offer practical starting points. The emphasis is on changes that fit your life, rather than a plan that only works on paper.
At ZEM Wellness Clinic Altea, assessment findings become a personal health blueprint, bringing together priorities, nutrition, movement, rest and follow-up. Carlos describes continued professional guidance as part of helping guests put that plan into practice at home.
The stay is a starting point, not an isolated experience. Mediterranean Longevity is an approach to build on, wherever you live.

I'm delighted to share this first episode of ZEM's podcast with you, Carlos.
Dr Carlos Gutiérrez
Thank you. I'm delighted to be here too.
Xilenia Zoghbi
Today we're discussing a fascinating and controversial topic that has become very popular. Everyone is talking about longevity. People keep asking us: "What does longevity actually mean?"
Dr Carlos Gutiérrez
Longevity is a concept, a term that's become very popular in a very short time, but its meaning has already evolved. If you look at the concept of longevity, the word itself means living longer, living a long life. But would you want to live a long life in poor health?
Xilenia Zoghbi
Absolutely not!
Dr Carlos Gutiérrez
I think that says a lot about what longevity means. This is how I explain it to people: as a doctor, my job isn't to make you live for years and years. My job is to help you live life to the full, right to the end. Usually, that goes hand in hand with living longer, but we should focus not on living for many years, but on living well and making the most of every day and every moment.
To me, that's longevity. We're also lucky to be in a setting where we practise what we call Mediterranean Longevity, a way of life. We're fortunate to be in the Mediterranean, but you don't have to be here to follow this lifestyle.
Xilenia Zoghbi
Really? What is Mediterranean Longevity like? What makes the Mediterranean different from the rest of the world when it comes to longevity?
Dr Carlos Gutiérrez
It's no secret. I think it's well known that this involves key elements such as nutrition and movement: moving from place to place, resting and being able to rest well. And not just that, but managing everyday stress and, of course, sleep.
And just as important is our connection with our community and other people: how you relate to your family, friends and those around you. That also seems to distinguish the Mediterranean region from other cultures and environments. All these factors play a very important part in longevity, in healthy longevity.
02:50
Xilenia Zoghbi
And for someone who wants to make this a goal in life, like me, for example, who wants to age well, am I already too late? I'm in my 40s. Am I too late, Carlos? Where should I start?
Dr Carlos Gutiérrez
It's never too late. In fact, studies tell us that whenever you start, you will always benefit. If you're changing your lifestyle and habits, working on your nutrition, movement, muscle mass, how you manage your mind or your rest, you will always benefit. Those benefits begin in the short term, whatever your age.
We start ageing at a very young age, so if we want to preserve the functions we have when we're at our best, in our youth, then clearly, the sooner we start, the better. But if we haven't, there's no need to feel guilty. It's never too late.
Xilenia Zoghbi
There's hope.
Dr Carlos Gutiérrez
Yes, of course. And it's hope backed by evidence. Does that mean saying, "Change your habits and we'll see whether you benefit"? Not at all. There will always be benefits.
For example, people say, or used to say, that we are what we eat. Some of us go further: "We are also what we absorb."
04:18
Xilenia Zoghbi
My goodness, tell us more.
Dr Carlos Gutiérrez
Following a healthy diet, such as the Mediterranean diet, which has proved to be one of the best dietary approaches in the world, with remarkable benefits in reducing cardiovascular risk, means making it a daily habit to eat seasonal foods. A largely plant-based diet, with an appropriate amount of protein, which may vary depending on your needs and stage of life.
Above all, a diet based on locally grown produce, without processed foods. Eat food that is as unprocessed as possible. You can cook and enjoy variety in your kitchen without turning to ultra-processed foods.
Xilenia Zoghbi
That's exactly what I wanted to ask you about, because so many diets have emerged over the years. Every five or ten years, there's a new diet that becomes a trend. Many diets cut out certain foods and replace them with others. Does the Mediterranean diet demonise any particular foods?
Dr Carlos Gutiérrez
No, it's not about demonising foods. If we want our diet to become a sustainable habit, it has to be something we can maintain over time and enjoy. It shouldn't be a sacrifice.
We should always aim for a realistic diet, one we can maintain and follow, one that makes sense and has been tried and tested over many generations. The Mediterranean diet is ideal for this. We know the Mediterranean diet is anti-inflammatory. Many of its effects, on digestion and cardiovascular health, and on the brain, come from its anti-inflammatory properties, from including antioxidants and olive oil, for example.
Xilenia Zoghbi
Liquid gold.
Dr Carlos Gutiérrez
Exactly. These components and ingredients have that beneficial effect. They also benefit the digestive tract. The digestive tract is home to both nerve cells and immune cells. All this is regulated simply by changing our diet.
Xilenia Zoghbi
So it's not about cutting foods out, but looking at where they come from, right? Choosing real food.
Dr Carlos Gutiérrez
I often tell my patients: you don't need to learn to read labels, you need to eat food that has no label. Because we need to get back to handling and cooking natural foods. Of course, that takes time. We need to make a habit of slowing down, learning and enjoying cooking. And, importantly, sitting at the table.
Xilenia Zoghbi
And sharing.
Dr Carlos Gutiérrez
Sharing, making space to connect with others over a pleasant meal. That also has a therapeutic effect.
07:48
Dr Carlos Gutiérrez
Here's a very simple example in nutrition. At Finca Althaya, our farm, we grow our own vegetables and fruit, all the produce you'll then eat at ZEM. This is extremely important because we oversee the process from the moment a plant starts growing until it reaches your plate.
Organically grown, of course, with attention to the season and which crops are planted next to each other. We work with people who understand this approach, who always keep a close eye on everything growing there and give it the utmost care.
Xilenia Zoghbi
It's fascinating you mention the farm, because there, as guests learn about the importance of eating real food, they can also help grow it and learn about the whole process. It's also a place, as you mentioned in your tips, where you can connect with nature.
Just imagine: we have 200,000 square metres. It's enormous. As well as areas for growing food, we have space for outdoor activities and yoga sessions at sunrise and sunset.
Dr Carlos Gutiérrez
Think of the benefit of experiencing, right where you see those vegetables growing, the joy of being in a clean environment, where you can enjoy outdoor activities, move around, practise yoga, run and be physically active. That's something we value very highly at ZEM. All our programmes include these kinds of activities. You have to experience it.
09:52
Xilenia Zoghbi
So, for someone pursuing longevity, what is the starting point? How can they begin to adopt this way of life?
Dr Carlos Gutiérrez
First, I'd recommend understanding your health. It's important to understand your health without becoming overly obsessive, but to identify your priorities and strengths, which aspects of your health you need to monitor more closely, and where you should take action sooner.
That requires the help of professionals who understand this whole-person approach to health, who can look at all the areas we've discussed that contribute to longevity, and establish a starting point and a plan. This is essential if we really want to make healthy longevity part of our lives.
Xilenia Zoghbi
For example, you come to ZEM, where we have the team to assess all aspects of your health. Once you have your assessment, what happens? What does the plan tell you?
Dr Carlos Gutiérrez
You'll be assessed by several professionals, each specialising in a different area. Not just the traditional areas or routine check-ups of the past, but now we also consider other factors, such as stress, the balance between your sympathetic and parasympathetic nervous systems, your gut health, your physical health and your functional age.
All very important. We use this to establish your plan. How can we improve and optimise function in each area? Sometimes, a normal result isn't necessarily the same as an optimal result. Our aim here is to optimise health. Even if you come with a specific goal, we always look at your health as a whole.
At the end of your stay, we'll give you a plan, a blueprint, which sets out your priorities and the follow-up needed for each of them. It includes personalised nutritional advice and, where needed, advice on supplementation. Of course, a physical activity plan, and a plan for mind-body balance and rest. All of this must be fully integrated.
And, very importantly, these steps should fit easily into your everyday life. If we create a plan that has nothing to do with your current life, we're unlikely to be very successful.
Xilenia Zoghbi
Right. It wouldn't be realistic. It looks great on paper, but I can't apply it to my routine. So you tailor it completely here.
Dr Carlos Gutiérrez
Exactly. You might live in a lovely village in the English countryside, or in the middle of New York. Clearly, the living conditions are completely different.
So, the professionals advising you, supporting you, your doctor, need to take that into account. They need to consider where you live and how you can make small changes each day, so it's easy to adopt this new approach.
14:02
Xilenia Zoghbi
Yes, so you build habits that last and become part of your everyday routine.
What about someone planning a specialist consultation to assess their health? As you said, first you assess where you are, then get your plan. But if they haven't been able to come yet, how about sharing some tips in this podcast to help them start exploring a Mediterranean Longevity lifestyle?
Dr Carlos Gutiérrez
First tip, as we've mentioned: change your diet if you're not already eating this healthily. Above all, look for unprocessed foods, foods you enjoy and find satisfying. Explore and try new flavours, always in moderation. Avoid large portions and too many snacks throughout the day.
Xilenia Zoghbi
Real food.
Dr Carlos Gutiérrez
That's essential. Exactly. Second: movement. If you drive to work, park a little further away and walk every day. Rather than setting yourself the goal of running marathons, just move more than you used to, more than your previous baseline.
It's also important to measure certain things we recommend, such as strength. Make sure you feel alive, strong and active, rather than slow. Slowness doesn't really go hand in hand with healthy longevity. In fact, we use certain measures to assess muscle strength, also focusing on those aspects.
Third: manage your stress. You need to make time to stop. Learn to pause. It's very important. We all experience stress. It's an inherent part of life. But what makes the difference is how you manage it.
Xilenia Zoghbi
Yes, when we're reactive, we overreact to whatever happens.
Dr Carlos Gutiérrez
It also depends on your stage of life. Think about how you felt at a job interview when you were 20, and how you'd feel at the same interview now. You'd handle it differently, wouldn't you?
Xilenia Zoghbi
Yes, as the years pass, you feel more in control. Knowledge helps with longevity too.
Dr Carlos Gutiérrez
Here's a tip I don't often give people, but I think it's another benefit and a lesson from the Mediterranean lifestyle.
Seek advice from people with more life experience than you: our older generations. People who have accumulated a great deal of wisdom, who have been through what you have, probably many times. That contact is very enriching and will help you learn. Not everything has to come from doctors.
You also need to connect with people who truly enrich your life, whether at home, with your partner, with friends, or with people you know bring a lot to your life. It is not just enjoyable and mutually enriching; it also keeps you active.
Having a conversation, joining a group with a shared interest, and sharing and exchanging opinions and ideas keeps your brain active and oxygenated. That's extremely important because, among the risk factors identified, one very important aspect is a sense of purpose: purpose in life, positivity, having something to work towards, to strive for and stay excited about. That's vital.
Xilenia Zoghbi
It's fascinating that you mention positivity. Even that affects longevity and this way of life.
Dr Carlos Gutiérrez
Undoubtedly. Some studies even link that attitude to increased cardiovascular risk , stress and even cognitive decline, for example. That's why it's important...
Xilenia Zoghbi
...to keep smiling.
Dr Carlos Gutiérrez
Exactly. It's all so important. Of course, it's easier in some environments than in others, but today we have technology available, and I think this could be one of its beneficial uses.
Rather than scrolling through things that don't interest you, use that technology to connect with people who share those interests...
19:52
Xilenia Zoghbi
What would Mediterranean Longevity look like as an experience at ZEM?
Dr Carlos Gutiérrez
At ZEM, you can enjoy the whole experience during a one- or two-week stay, starting with a very precise health assessment. Your nurse, doctor, personal trainer and wellness adviser will look at every aspect of your health and personalise your programme and interventions, from your menu and diet to your fitness training, wellness treatments and outdoor activities.
At the end, you won't leave with a series of reports, each containing separate recommendations. Your doctor will prepare a blueprint bringing together all the information gathered during your stay in a single document, setting out a plan you can put into practice from the day you leave ZEM.
It will include recommendations on medical tests, interventions, supplementation and treatment. It will also set out priorities: what to do and when, helping you introduce those small changes into your daily life, step by step.
Xilenia Zoghbi
So the blueprint is like taking ZEM home. It stays with you.
Dr Carlos Gutiérrez
Exactly. We don't want the experience to end at ZEM. We want this to be the starting point for supporting you as you make changes to your health.
Evidence shows that when you want to make changes in your life, you're much more likely to succeed with support, with someone alongside you, a professional who can guide you through the process, answer your questions, help you work through difficulties and always be there.
Xilenia Zoghbi
We feel less accountable to ourselves than to the person supporting us. So you have that sense of commitment.
Dr Carlos Gutiérrez
That commitment always helps to keep you moving forwards.
22:23
Xilenia Zoghbi
We could spend so much longer talking about longevity. Time's running away with us. I think we've reached the end, but it's been a real pleasure spending this time with you, Carlos, and sharing valuable information with everyone listening who wants to begin this way of life.
Dr Carlos Gutiérrez
Likewise, it's been a pleasure. And that's the key message: we're not talking about anything very complicated. First, we need a foundation, then we build on it, refine it and look at the finer details of medical care, nutrition and supplementation.
But the foundation, that way of life with strong scientific support and a very high level of evidence, isn't difficult. You can apply it wherever you are. That's the message to take away.
Xilenia Zoghbi
Thank you.
Dr Carlos Gutiérrez
Thank you too.

Xilenia Zoghbi
